11:50 AM CASE STUDY: Using Digital Health to Enable Integration for Elderly Residents in Aged Care Facilities
Secure exchange of health information is a core foundation of Australia’s expanding digital health program. This is in response to the GRACEMED study that identified less than 10% of residents’ medication charts matching their GP’s records. SNPHN is exploring ways in which My Health Record (MyHR) and secure message delivery (SMD) could be used to facilitate the increasingly complex nature of care in Residential Aged Care Facilities (RACFs) and to support clinical handover to improve patient safety.
Cynthia will provide insight on their survey and interviews with 103 local RACFs and their findings, with 56% of participants indicating they would benefit from more knowledge regarding the use of Digital Health Technology (DHT) to support medication management; improve clinical handover; reduce workflow duplication; and provide accurate and up to date information for sharing with other healthcare providers.
