NSW Ambulance has led the way with the development of Authorised Care Plans and Authorised Palliative Care Plans (both Adult & Paediatric) a project initially trialled with Sydney Children’s Hospital Westmead and the Aged Care Service in Southern NSW. The provision of integrated care within NSW Health is increasingly important given the aging population and the growing demand on the Health Service. Now, more than ever, all service providers, including NSW Ambulance, need to be working together to ensure that every opportunity is identified to improve outcomes for patients & increase efficiency for the Health Service.

In this session, Colleen will outline the benefits, implications and challenges of these Authorised Care Plans, how they facilitate better engagement andcollaboration with the Aged Care Sector.
